Please complete this grant application electronically. Save your file as and forward to your school principal for approval. Principals should Email completed applications directly to grants@panhandlealliance.org. Original signatures will not be necessary if electronically submitted through Email by principals.

Electronic submissions are preferred; however, if you must submit in hard copy form, signatures are required. Mail completed application with signatures to:

Grant Payment

The process for grant payment utilizes the familiar LPOSD 84 Requisition for Purchase Order forms.  Follow these simple steps to purchase materials for your grants. 
 
1.  Fill out the standard LPOSD 84 Requisition for Purchase Order Form
(yellow and white copy) including Vendor Information, Date, Ship-to.  Identify your PAFE grant number on the first line of the “Budget Account Number” column. Add all items for purchase per your approved PAFE grant including: Description, Part #, Unit, Unit Price, Quantity, and Total.  Complete the form with your signature next to “Requestor” at the bottom of the form. 
 
2. Mail your completed form to the PAFE Office for approval by the PAFE Compliance Officer.
